To film in any of the catholic cathedrals, churches or basilicas in Italy, a written request, complete with script, a list of the crew members must be submitted to the Vicariate of Rome:

It is advisable to request authorization several weeks in advance, and to avoid concentrating filming over the Christmas and Easter holidays and on weekends.
The Ministry of Internal Affair, through the internal office Direzione Centrale per l'Amministrazione del Fondo Edifici di Culto (FEC), manages and controls more than 700 churches and other religious properties all over Italy. If the property where you want to film falls under jurisdiction of the FEC, then request must be sent to:

The issuing of the authorization can be subject to authorization by the Vicariato too (that depends on the subject and the type of production). The relevant fees and the authorization are determined on a case by case basis, according to the subject matter of the project, the kind of production (feature film, commercial, documentary, etc) and the length of the shoot (1/2 day or more).
To shoot in or around hospitals, it is necessary to contact the individual administration of local health care companies whose directories and addresses are available at the Public Relation Offices of Health Care Units of the individual regions.
You should send your request either to the director or the PR department of the hospital.
Requests should not entail situations that might disturb patients. Authorization for chronicles, scientific documentaries, and news reports can be obtained easily and quickly. For feature films or commercials it is a good idea to submit applications as much in advance as possible in order to make the necessary arrangements and be sure of obtaining the permits in time for your production.

In Italy, it is not permitted to film military personnel.
The Military Forces (air, land and sea), through the Ministry of Defense, can authorize filming in many locations throughout Italy.
However, it is impossible to list them in advance, nor to set a definite timeframe for obtaining permits.
The requests are examined on a case-by-case basis. The more comprehensive your application, the faster it will be processed.
